Does It Build Up If You Take It Daily?
This is the first question worth separating out, because "every day" only becomes a real concern if each dose is still around when the next one arrives. For methylene blue, it mostly is not. In a study of seven volunteers given 100 mg, blood levels after intravenous dosing followed a multiphasic curve with an estimated terminal half-life of about 5.25 hours. The same study found that oral dosing produced a much lower area under the curve than intravenous dosing, meaning less of an oral dose reaches the bloodstream in the first place.

The caveat: blood levels are not the whole story. Methylene blue distributes into tissue, and the same study found different organ distribution after oral versus intravenous dosing in animals. "It clears from blood overnight" is not the same claim as "nothing accumulates anywhere," and the second claim is one nobody currently has the data to make.
What the Long-Term Human Data Actually Shows
Here is the honest state of the evidence, because it is easy to find confident answers online that the research does not support.
The human studies on low-dose methylene blue are mostly single-dose. In a randomized, double-blind, placebo-controlled imaging study, a single low oral dose modulated task-related and resting-state brain networks in healthy adults. That is a real finding, and it is a finding about one dose on one day. It does not tell you what happens on day 200.
The longest controlled exposure comes from a different compound at much higher doses. A 15-month phase 3 trial randomized 891 patients with mild to moderate Alzheimer's disease to LMTM, a stable reduced form of the methylthioninium molecule, at 75 mg or 125 mg twice daily. The prespecified primary analyses were negative. On the safety side, gastrointestinal and urinary effects were the most common adverse events and the most common reasons people stopped, and dose-dependent reductions in hemoglobin were the most common laboratory abnormality.
Two things follow from that trial, and it is worth holding both. It is the clearest evidence that people can take a methylthioninium compound daily for over a year under medical supervision. It also shows the effect that supervision was watching for: a blood-related, dose-dependent one. Supplement servings are far below 150 to 250 mg per day, and LMTM is not the same molecule sold as methylene blue, so this is context rather than a green light.
What is missing is the study you actually want: healthy adults, supplement-range doses, taken daily, followed for a year, with blood work. It has not been done. Anyone telling you daily low-dose use is proven safe long term, or proven harmful, is going past the evidence in one direction or the other.
Why the Dose Matters More Than the Frequency
Methylene blue does not follow a more-is-better curve. In animal work, low doses facilitated memory and increased brain oxygen consumption while higher doses did not, and reviews of its neurometabolic activity describe the same hormetic pattern, where the useful range sits inside a window rather than climbing with dose. At higher concentrations the molecule's behavior changes character entirely.
Frequency does not get you around that. A daily habit at a sensible amount and a daily habit at three times that amount are not the same experiment, and the second one is not a more committed version of the first. This is also why label accuracy matters so much in this category: if the capsule does not contain what it says, you are not controlling the one variable that actually determines the outcome.
There is a second, sharper reason the amount matters. Methylene blue is a potent inhibitor of monoamine oxidase A, and that inhibition is dose-dependent. Should You Cycle Off?
Cycling schedules circulate widely in this space: five days on and two off, three weeks on and one off, a week off each month. It is worth being direct about where those come from. No published study has tested one schedule against another and shown that any of them is better. They are conventions borrowed from how people cycle other supplements, not findings.
That does not make breaks pointless. The practical arguments for them are decent ones:
- A break is the only honest test of whether it is doing anything. If you stop for a week and notice nothing, that is useful information about a product you are paying for every month.
- It creates a natural checkpoint. A scheduled pause is a good moment to revisit the decision with your provider rather than letting a daily habit run unexamined for a year.
- It lowers total exposure. Given that the long-term data does not exist, less total exposure for the same perceived benefit is a defensible preference rather than a superstition.
