It started life as a dye for cloth.

In 1876, a chemist named Heinrich Caro created a new blue dye for wool at BASF in Germany. A year later it became the company's first patent for a coal tar dye.1

That is where the story begins, and it is also why the grade on the label still matters. The same molecule is made today for dyeing, laboratories and aquariums, and none of those grades is meant to be swallowed.

It showed scientists what was inside a cell.

Methylene blue clings to some parts of a cell and ignores others, so under a microscope it made invisible things visible. By 1886, Paul Ehrlich was using it to stain the parasite that causes malaria.1

Watching a dye find one target and leave the rest alone gave Ehrlich an idea that shaped modern medicine.

The idea it gave him

If a dye could seek out one kind of cell, Ehrlich reasoned, a compound could be built to seek out a disease. He spent the rest of his career chasing what he later called a "magic bullet."

Five years after that first stain, methylene blue became his first attempt at one.

It became the first synthetic drug ever given to a patient.

In 1891, Ehrlich and Paul Guttmann treated two patients with malaria in Berlin using methylene blue. It is generally credited as the first fully synthetic compound ever used in clinical medicine.2

Before that moment, medicines came from plants, minerals and animals. After it, chemists knew they could build one.

Why it didn't stay a malaria drug

Quinine was already available, and it eventually took over. But methylene blue kept shaping medicine anyway.

Chemists studying its structure found a compound called phenothiazine, which became the starting point for some of the first antipsychotic medications and the tricyclic antidepressants.8 That family connection is exactly why methylene blue must never be taken alongside antidepressants.7

It went to war in the South Pacific.

During World War II, Allied troops in the South Pacific took methylene blue to prevent and treat malaria.8 It came with a side effect every soldier noticed.

It turned their urine green and the whites of their eyes blue. The side effects were harmless, but soldiers noticed, and disliked them enough that methylene blue eventually fell out of favor as an antimalarial.8

It went from the battlefield to the operating room.

In 1933, physicians documented methylene blue reversing methemoglobinemia, a blood condition where red blood cells cannot carry oxygen properly.5 Within a few years it was the standard treatment, and it still is. Surgeons started using it as a dye too, tracing lymph nodes and locating the parathyroid glands during operations, because it stains some tissue and not others, the same property Ehrlich noticed back in 1886.

Is that still how hospitals use it today?

Yes. In 2016, the FDA formally approved a branded injectable version, ProvayBlue, for the methemoglobinemia use, the first approved therapy for that specific condition.5 The underlying use in hospitals goes back decades earlier than that approval.

Can I take this with antidepressants?

No. Methylene blue has monoamine oxidase inhibiting activity, and combining it with SSRIs, SNRIs, MAOIs, or other serotonergic drugs carries a risk of serotonin syndrome, which is serious. This is the single most important contraindication and we'll not soften it. If you take any antidepressant, don't use this product without your doctor’s explicit direction.

Is it safe if I have G6PD deficiency?

No. People with glucose-6-phosphate dehydrogenase (G6PD) deficiency should avoid methylene blue, as it can trigger hemolysis. If you don’t know your G6PD status and have a family history, ask your doctor for a test before using.

Can I take it while pregnant or nursing?

No. Methylene blue isn't recommended during pregnancy or while breastfeeding. Please speak to your OB or healthcare provider.

Can I give this to my kids?
